If you are ready to join an amazing team of passionate behavioral healthcare professionals, then we invite you to apply to this opportunity!_ *Real Recovery of Asheville is looking for a Aftercare Coordinator! * *Responsible for ensuring clients leave treatment with a solid aftercare plan, safe housing and further needed treatment.
* *CADC R and higher Preferred* *Essential Duties and Responsibilities* • Provides quality direct and indirect client care to individuals with substance abuse or chemical dependency problems.
• Assess clients continuing care/referral needs.
• Establishes rapport with clients.
• Obtains the necessary information and releases from client, family, referral sources and other appropriate resources.
• Makes referrals to Mental Health and Drug & Alcohol facilities for continuing care.
• Sets up Halfway House interviews and placements.
• Works closely with counselors and members of multi-disciplinary team.
• Attends and participates in program, clinical and administrative meetings.
• Actively supports the facility’s ongoing referral program.
• Participates in mandatory orientation and training activities.
• Other duties as assigned.
*Required Qualifications* • High School diploma required.
Bachelor’s Degree in a human service related field preferred.

• Excellent oral and written communication and interpersonal skills • Knowledge of clinical levels of care, primary funding streams and referral sources • Ability to problem solve by gathering and analyzing information • Ability to handle a crisis situation and react appropriately • A working knowledge of federal and state standards as well as regulating body standards.
