Primary Purpose of Organizational Unit:
The Community Alternatives Program for Disabled Adults (CAP/DA) is a special Medicaid program under the Community Care Section of the Division of Medical Assistance.
The program makes care at home a real possibility for many people who face nursing home placement.
When the program began in 1982, four counties participated.
It is now available in all 100 counties.
This program will be under the Social Work Division within the agency.
Primary Purpose of Position: The Social Worker in this position is responsible for coordinating services through the Community Alternatives Program (CAP) to clients, their families and caregivers.
This Social Worker will be responsible for a determined case load and be responsible for conducting home assessments, developing and managing a plan of care for each client and to also monitor each client on a regular basis.
The Social Worker will complete all required documentation and detailed correspondence.
Work Schedule: Regular work hours are 8:00 a.
m.
to 5:00 p.
m.
, Monday through Friday.
Description of Responsibilities and Duties Conduct home assessments Develop plan of care with client and family representatives Manage the plan of care Coordinate services of CAP Providers Ensure Medicaid regulations are followed Complete all required documentation and correspondence Make decisions regarding assessments and services Provide direction or instruction to lower level staff, volunteers or care providers Work with clients, families, habilitation/treatment teams in the development and implementation of treatment plans Maintain contact with other professionals required in the coordination of services, provision of program orientation and consultation to the community.
Qualifications Required Minimum Training: Bachelor"s degree in social work from an accredited school of social work; OR Bachelor"s degree in a human services field from and accredited university and one year directly related experience; OR Bachelor"s degree from an accredited college or university and two years directly related experience Additional Training/Experience: The Social Worker should be flexible in that he/she attends training as required and needed to do the job.
Knowledge, Skills and Abilities Special Requirements - Considerable knowledge of social work principles, techniques, and practices and their application to individual casework, group work and community problems.
Knowledge of the psychosocial, socioeconomic and behavioral problems and their treatment.
Knowledge of governmental and private organizations and community resources.
Knowledge of laws, regulations and policies which govern the program.
General knowledge of medical terminology, disease processes and their treatment is required in certain programs or settings.
Skill in establishing rapport with a client and applying techniques of assessing psychosocial, behavioral, and psychological aspects of client"s problem.
Ability to establish and maintain effective working relationships with administrative superiors, members of case load and their families, and with civic, medical, social and religious organizations.
Ability to train or instruct lower-level social workers, students, or interns.
Ability to express ideas clearly and concisely and to plan and execute work.
Physical Requirements Position requires mostly sedentary work that will occasionally require walking or standing for brief periods of time.
Requires exerting up to 10 pounds of force on a recurring basis as well as routine keyboard operations.
The job requires normal visual acuity and field of vision, hearing and speaking ability.
The position risks no exposure to environmental hazards.
Physical Effort: Great physical effort is not necessarily a part of the job, but long hours to attend evening meetings or to work on agency related matters may be required.
Occasional travel out of town may also be necessary.
Stamina is required to endure and resolve crisis oriented work situations.
